Soccer Camp Application

A $75.00 deposit is due by June 15, 2008.
T-Shirt and ball cannot be guaranteed for applications received after deadline.
Balance is due one week prior to the start of camp.
No refunds once camp has started.
Note- Acceptance is based upon available space.

Application Information-

By signing this application, I certify that my child is in excellent physical health and may participate in strenuous physical activities, including soccer.

I agree to waiver or release Panther Soccer Camp, anyone associated with the camp, Whitman-Hanson Regional High School, Whitman Recreation Commission, and the Town of Whitman from any and all liability for any injuries or illness sustained while attending camp.

I also agree to assume financial responsibility for any personal injury or negligent act by my child while attending Panther Soccer Camp.

Permission is also granted to Panther Soccer Camp to use any pictures of my child for use in promotional brochures and/or camp web site.
